Please be sure that you submit your enrollment within 31 days of your hire/rehire date. If you do not finish your enrollment or stop mid-way you will have a pending notification on the Enrollment section.
If your enrollment is successful, a green check mark with a “Submitted” notification will appear in the Enrollment section. You will also see a summary of your benefit elections at the end of the process.

When you experience a life event; such as marriage or change in benefit coverage due to change in spouse's employment, you have 31 days from the life event to change your HMC benefits.
To change to your benefits, send the completed form and supporting documents to benefits@hmclause.com.

To be eligible, you must have been living together on a continuous basis for at least twelve (12) months prior to the date of this affidavit and submit proof of qualifying cohabitation.
Details on qualifying cohabitation includes one separate proof of cohabitation and one separate proof of financial interdependence. Proof may be one document with both names or two separate documents that show the residence of each partner.
